Your First Visit
Please arrive about 10 minutes early so that you have enough time to fill out a new patient form below, or save time by filling out the form in advance by clicking the link below.
Initial Examination
Your dentist will review your medical and dental history and then complete a comprehensive dental exam. The exam will consist of an evaluation of your gums, teeth, bite, and jaw, an oral cancer screening and any required X-rays. A fluoride rinse may also be administered to help strengthen your teeth and lower your risk of tooth decay.
Treatment Plan
Your dentist will develop a long-term dental treatment plan customized to your needs based on your examination, dental history, and oral health goals.

We do our best to help you navigate our options for payment and insurance so you can focus on more important things, like restoring your oral health and achieving your dental goals.

We accept debit and most major credit cards. Payment is due on the day the service is rendered.
Insurance
Payment is due at the time of your appointment. If you would like to learn more about your insurance plan, our friendly administrative staff would be happy to take a look with you at your appointment and answer any questions you may have regarding submitting your claims.
